Primarni aldosteronizam - novi pogledPrimary aldosteronism – a new perspective

Primary aldosteronism is present in 5-13% of the population with hypertension. Currently accepted screening method is determination of aldosterone-renin ratio. Although this screening test has not been standardized, most authors use the cut off value of 20-40 ng/dl / ng/ml/h. The diagnosis of hyperaldosteronism cannot be based only on increased aldosterone-renin ratio but requires independent verification of autonomous hypersecretion of aldosterone. For that purpose, verification tests are used, followed by morphological diagnostic methods. Computer tomography and magnetic resonance imaging cannot discern between unilateral and bilateral disease in 37.8% of the cases. Therefore, catheterisation of adrenal veins and determination of both aldosteron and cortisol blood levels is the most reliable method of determining whether the disease is unilateral or bilateral. The treatment indicated in patients with aldosterone-producing adenoma or unilateral adrenal hyperplasia is unilateral laparoscopic adenalectomy. Patients with bilateral disease are treated by mineralocorticoid receptor antagonists.
